He had many afflictions that had never before been documented, sato masochistic things. The needle x-ray picture is an example of the punishment he did to himself for the bad things he was doing. Perhaps the most horrible thing was his compulsion to write to the child's mother with the details of the crime years later. But luckily the mother could not read so she was spared that shock.
