Tips on Answering Delegation Questions on The Hesi Exit Exam
Management questions on the HESI exit exam include questions of nursing delegation amongst other things. Here are a few ideas on how to answer thos delegation questions and improve your chances of passing the HESI exit exam.
First and foremost you have to know who the registered nurse can delegate to, what can be delegated and the criteria for making a delegation to a particular person. The nurse can delegate to an unlicensed assisstive personnel UAP like the certified nurse assistants CNA or the license practical nurse. Before making a delegation, the RN must make sure the person she is delegating to has the necessary experience or instruction to carry out the delegated task and must also be instructed if anything out of the ordinary occurs. When answering the HESI question always keep in mind that if anything obvious goes wrong the person being delegated to has to report back to the RN.
Now when delegating to the CNA, you can give them tasks that fall within the implementation part of the nursing process. The UAP can take vital signs, perform range of motion excersices on the patient, measure intake and output and transport patients. They cannot give medication or assist physicians during invasive procedures and cannot perform sterile or invasive procedures. One tip for the HESI exam here is thhis: if you are not sure of what tasks should be assigned to whom, always narrow down your answers to the ones that give vital signs to the UAP.
The LPN on the other hand can do a lot more than the UAP. When deciding on which answer to pick in a case with the LPN on the HESI exam, keep in mind that the LPN can give medication, perform wound dressing changes and assist the physician during invasive procedures. The LPN cannot hang intravenous medication IV. The LPN cannot admit or teach patients.
The graduate nurse student even though she is an RN cannot do everything that an experienced nurse does. So when answering delegation questions about the new RN graduate, always pick choices where the RN gets a patient who is very stable and does not require any complex or specialized are. Do not pick patients who are being discharged or who have been newly diagnosed with a disease because they need teaching which should be done by the experienced RN.
When selecting your choices on delegation remember that the RN should always without exception perform admission assessments, do patient teaching, assess patients and plan patient care to the point of discharge planning.The RN should also create the care plan for the patients.
