Restoring Your Drivers License After a DUI or DWI in Bradford County, PA

Get informed on what you need to do to restore your driving privilege’s after a DUI or DWI, with form numbers and more.

In order to wave the Interlock Device you must have NO vehicles in your name!

You must also fill out the Self-Certification of vehicle(s) owned/operated form. the number for this form is DL-21sc. On that form you must state that you do not have a vehicle in your name.

Even though you may choose to wave your right to drive for another year so you do not have to put the Interlock Device on your car (remember you can not have a car in your name) you still must send in this form with a check or money order.

Please do not just say to yourself “I just won’t go in to get my license right now” “I will wait another year then go and get my license”

This will not work!

Your year will not start until you have done all the forms that are required of you and sent in the required fees with all the forms.

Also if you had a CDL when you received your DUI or DWI you will need to get an application for revision/ return of your CDL classification form. This form is only needed if you choose to return your CDL the number for this form is DL-100CD. You will need to fill out this form and send a check for $26.00 to the same Harrisburg address listed above.

All the forms that are listed above can be found at www.state.pa.com

There are many different forms on this website that may or may not pertain to you so please be sure that you copy the correct forms that you need, if not you will slow up your process and the required waved year will not start until all forms are done correctly and filed in Harrisburg.
